Retailers must complete the following forms each year.
Renewals are processed in May. Applications are due by April 15th.
AB-100 - Alcohol Beverage Individual Questionnaire
AB-101 - Alcohol Beverage Appointment of Agent
AB-200 - Alcohol Bevarage Licenses Application
The AB-220 only needs to be completed by retailers wishing to sell on a temporary basis. AB-100 and AB-101 must accompany the AB-220 application
